When a new mother returns home from hospital one of the team will arrange a visit at home the next day. Further visits will then be arranged and approximate times will be given. Patients may be referred to a District Nurse by anyone but usually by a doctor, hospital or other healthcare professional.
Community Health Visitors
Health Visitors are qualified Specialist Health Practitioners working in an advisory role to families to promote good health practice and prevent ill health. Their specific role is supporting parents in the care of their pre-school child. The role includes seeing families at home or in the surgery to discuss aspects of child and parental/carer health including: feeding, sleeping, adult well-being, family health, growth and development.
